برای تشخیص عفونت هلیکوباکترپیلوری راه های مختلفی وجود دارد اما نکتهی مهم این است که کدام راه تشخیصی قطعی تر است و به نفع بیمار؟
روش اول و ارجح از نظر فوق تخصص های گوارش روش اندوسکوپی است :
در روش اندوسکوپی یک اندوسکوپ از راه دهان وارد معده شما می شود که دوربینی دارد و پزشک می تواند از طریق این دوربین معده و مری را به وضوح ببیند. اگر زخم و التهابی وجود دارد آن را مشاهده کند. حتی می تواند ابتدای روده بزرگ رت نیز بررسی کند بنابراین اندوسکوپی تنها مختص مشاهده معده نیست.
در این روش پزشک بعد از مشاهده مری و معده و روده ، از مناطقی از معده که در مشاهدات التهاب یا زخم دارند نمونه بافتی تهیه میکند تا بررسی شود. تهیه این نمونه برای بیمار کاملا بدون درد است. این نمونه از این لحاظ اهمیت دارد که پزشک مستقیما با دیدن تغییرات، از خود تغییرات نمونه میگیرد که احتمال خطا در جواب آزمایش را شدیدا کاهش میدهد و دوم اینکه میتواند خوش خیم یا بدخیم بودن زخم را نیز نشان دهد و پزشک می تواند بهترین تشخیص و بهترین درمان را برای شما انجام دهد. به همین دلیل این روش ارجح است.
در این روش نمونه بافتی گرفته شده به دقت بررسی می شود. آزمایش بافت شناسی یکی از روش های قابل اطمینان جهت تشخیص و شناسایی هلیکوباکترپیلوری مطرح می شود .به کمک این روش به راحتی می توان به خصوصیات مورفولوژی و وضعیت باکتری پی برد و تراکم بالای آنرا در جمعیت مشخص نمود . بدلیل حضور بالای فلورمیکروبی در روده استفاده از روش های تشخیصی ویژه ای برای تائید حضور هلیکو باکتر پیلوری الزامی است .
روش بعدی تشخیصی برای هلیکوباکترپیلوری آزمایش خون است که حضور آنتی بادی ضد هلیکوباکترپیلوری را در خون نشان می دهد لذا ممکن هست حتی تا ماه ها بعد از عفونت نیز ، این آزمایش مثبت کاذب را نشان دهد و از لحاظ علمی برای افرادی که قبلا سابقه ی یک بار درمان هلیکوباکترپیلوری را داشته اند ممکن است همیشه مثبت باقی بماند. ضمن اینکه خوردن آنتی بیوتیک ها نیز جواب را ممکن است اشتباه کند.
روش دیگر آزمایش اوره تنفسی است که خیلی اختصاصی نیست و میزان اوره را در تنفس شما چک میکند و چون اوره با رادیواکتیو نشان دار شده است برای افراد باردار مناسب نیست ضمن اینکه احتمال خطای بالایی دارد و اصلا قابل اعتماد نیست.
روش بعد آزمایش مدفوع است که وجود آنتی ژن سطحی باکتری هلیکوباکتر پیلوری را در مدفوع نشان می دهد که بازهم احتمال خطای زیادی دارد و خوردن آنتی بیوتبک ها جواب این آزمایش را اشتباه می کند.
بنا براین هم اکنون روش ارجح در ایران اندوسکوپی و نمونه گیری با اندوسکوپ است.
