هلیکوباکتر پیلوری متهم ردیف اول زخم معده است!

ولی باید گفت مهم ترین عامل ایجاد کننده زخم معده، باکتری هلیکوباکتر پیلوری شناخته شده است.

“تنها باکتری‌ای که می تواند در محیط خشن معده و با حضور شیره معده، رشد نماید، هلیکوباکتر پیلوری می باشد”

هلیکوباکتر پیلوری با مصرف غذا یا آب آلوده از راه دهان وارد معده می شود و کافی است یک بار بتواند در موکوس معده پنهان شود.
در این حال باکتری مذکور در لایه مخاطی معده مستقر شده و توسط تاژک های برآمده، خود را در میان سلولهای لایه مخاطی جای می دهد و یا در صورت بروز زخم معده، در ناحیه فاقد لایه مخاطی و در واقع بر روی سلولهای اپی‌تلیوم معده استقرار یافته و تاژک های خود را در آن ناحیه فرو می برد.

برای مقابله با هلیکوباکتر پیلوری، سیستم ایمنی بدن فعال می شود. سلول های سیستم ایمنی به علت موکوس ضخیم معده، به آسانی به این ناحیه نمی رسند.

در واقع هنگامی که لایه موکوسی نازک شده و گلبول های سفید وارد این ناحیه می شوند، نمی توانند از آن خارج شوند.

به سبب درگیری سلول های ایمنی با هلیکوباکتر، آنتی بادی های موجود در خون زیاد می شوند.

اگرچه در روند مذکور، خود هلیکوباکتر پیلوری ممکن است عامل ایجاد زخم نباشد، اما التهاب لایه آستری معده که توسط هلیکوباکتر ایجاد می شود، زمینه ساز بروز زخم یا حتی ایجاد سرطان معده می باشد.

برای تشخیص عفونت هلیکوباکترپیلوری راه های مختلفی وجود دارد اما نکتهی مهم این است که کدام راه تشخیصی قطعی تر است و به نفع بیمار؟

روش اول و ارجح از نظر فوق تخصص های گوارش روش اندوسکوپی است.

در این روش نمونه بافتی گرفته شده به دقت بررسی می شود. آزمایش بافت شناسی یکی از روش های قابل اطمینان جهت تشخیص و شناسایی هلیکوباکترپیلوری مطرح می شود .به کمک این روش  به راحتی می توان به خصوصیات مورفولوژی و وضعیت باکتری  پی برد و تراکم بالای آنرا در جمعیت مشخص نمود . بدلیل حضور بالای فلورمیکروبی در روده استفاده از روش های تشخیصی ویژه ای برای تائید حضور هلیکو باکتر پیلوری الزامی است .
